The NPMA-33 records evidence of wood-destroying insects — termites, carpenter ants, powderpost beetles, wood-boring beetles — along with visible damage and any prior treatment. Completing it usually requires a pest control licence, so many home inspectors either hold that licence separately or coordinate with a licensed operator. The form covers evidence observed in accessible areas only, and it is a report of observation rather than a warranty. It is commonly requested for VA loans and in regions with high termite pressure.
